Before You Enroll: Verify Your Eligibility
Before searching for D-SNP plans, confirm you meet both eligibility requirements:
- Medicare Part A + Part B: Check your Medicare card — it should show your coverage start dates for both Part A and Part B.
- Medicaid enrollment: You should have received a Medicaid ID card or letter from your state. If unsure, contact your state Medicaid office or call 1-800-MEDICARE to verify.
When You Can Enroll
One of the biggest advantages of D-SNPs is that dual-eligible individuals have far more enrollment flexibility than the general Medicare population:

Step 2: Compare Your Options
Once you have a list of available D-SNPs, compare them on these key factors:
- Premium: Most D-SNPs have $0 premiums, but confirm.
- Drug coverage: Enter your medications into the plan’s formulary checker to see exactly what you’ll pay.
- Provider network: Confirm your doctors, specialists, and preferred hospitals are in-network.
- Supplemental benefits: Compare OTC allowances, dental maximums, vision benefits, food allowances, and transportation.
- Star rating: CMS rates plans 1–5 stars based on quality. Choose 4 or 5-star plans when possible.
Step 3: Gather Your Documents
Before enrolling, have these on hand:
- Your Medicare card (shows your Medicare Beneficiary Identifier/MBI)
- Your Medicaid card or Medicaid ID number
- Your Social Security number
- Your current address and date of birth
- A list of your current medications (name, dosage, frequency)

- Full dual eligibles can switch at any time — changes take effect the first of the following month
- Partial dual eligibles can switch once per quarter (January, April, July, October)
